Healthcare Reserves has recently partnered with one of Cleveland's well respected organizations that services the post-acute population with a strong focus on home health and hospice services.  Our customer is looking for a Licensed Practical Nurse - LPN - to provide direct patient care under the direction of a designated RN Care Manager. LPN Responsibilities and Highlights of the Position:

  • Mileage Reimbursement on top of competitive hourly rate for home care services.
  • Provides prescribed medical treatment and personal care services to patients in a home or inpatient setting.
  • Administers specified medication, orally or by subcutaneous or inter-muscular injection, and notes time and amount on patients' charts.
  • Provides wound care
  • Reinforces instructions and explanations of the patient Plan of Care or teaching plan.
  • Prepare and administer injections and other treatments as needed.
  • Reports any significant changes in the patient’s health status to the appropriate individual(s).
  • Completes all required forms, reports, time cards, itineraries, etc, accurately and within designated timeframe.
  • Performs other duties as assigned.

 

Working Conditions:

  • Travel by car throughout Cuyahoga County
  • Exposure to changing weather conditions; sometimes driving in inclement weather
  • Varied residential environments, based on assigned caseload
  • May be exposed to resident waste, tissue and blood products; required to use personal protective equipment and universal precautions accordingly
  • May be exposed to infectious pathogens, virus and disease
  • May be exposed to combative patients and/or patients with delirium/confusion

 

  • High School Diploma or GED.  Graduate of an accredited LPN program.  Associate's degree in Nursing preferred.
  • One year of hospital experience.
  • Home care and/or Community Health experience is preferred.
  • Ability to work independently with minimal supervision.
  • Demonstrated ability to maintain strict confidentiality.

Required Licenses, Registrations and/or Certifications:

  • Nursing certification; licensed in the State of Ohio.
  • CPR certification is required.
  • Valid driver's license.

Physical Requirements:


  • Close and mid-distance visual acuity.
  • Ability to remain in stationary position for extended periods of time
  • Ability to carry 15 pounds while walking or climbing stairs
  • Ability to squat, kneel, bend, reach and grasp (while bending) to assist a 250-pound patient to their feet from a sitting position and to assist the patient back to a sitting position
